There’s a well known comic drawing out and about how things will often get confused between the customer ordering something, and the supplier delivering it.
Which is what popped into my head when I read this little story.

What the customer wanted: small beads that are coated in non-toxic glue that sets when wet.
What the supplier delivered: small beads that, when swallowed, mentally feck up the swallower and leave them in hospital with interesting side effects.
I’d say something about ‘heads should roll’ over this, but the Chinese have form with taking such phrases literally…
